By using the services of Mobile Diagnostic Laboratory, Inc. ("MDL," "we," "our," or "us"), you agree to be bound by these Terms of Service. If you do not agree to these terms, please do not use our services.

2. Services Description

Mobile Diagnostic Laboratory provides mobile point-of-care testing, laboratory services, fingerprinting, background checks, and DNA testing services. We bring diagnostic services directly to your home, office, or facility in Southeast Arkansas and surrounding areas.

3. Patient Responsibilities

As a patient or client, you agree to:

• Provide accurate and complete personal and health information
• Present valid identification when required
• Follow preparation instructions for tests (such as fasting requirements)
• Pay for services as agreed, including copays and deductibles
• Notify us of any changes to your contact or insurance information

4. Appointments and Cancellations

We request at least 24 hours notice for appointment cancellations. Repeated no-shows or late cancellations may result in a cancellation fee. We reserve the right to reschedule appointments due to unforeseen circumstances.

5. Payment Terms

We accept Medicaid, Medicare, and most major insurance plans. Payment is due at the time of service unless other arrangements have been made. You are responsible for any amounts not covered by insurance.

6. Test Results

Test results are typically available within 24-48 hours. Results will be communicated to you and/or your designated healthcare provider. We are not responsible for delays caused by third-party laboratories or circumstances beyond our control.

7. Limitation of Liability

Mobile Diagnostic Laboratory provides testing services but does not provide medical diagnoses or treatment recommendations. You should consult with your healthcare provider regarding test results and any medical decisions. Our liability is limited to the cost of the services provided.

8. CLIA Certification

Mobile Diagnostic Laboratory is CLIA certified and operates in compliance with all applicable federal and state regulations governing clinical laboratories.

9. Modifications to Terms

We reserve the right to modify these Terms of Service at any time. Changes will be effective upon posting to our website. Continued use of our services constitutes acceptance of the modified terms.
